Antibiotic resistance is one of the major challenges in the treatment of infectious diseases, and its spread can reduce the effectiveness of antimicrobial drugs. One of the most important mechanisms of resistance in Gram-negative bacteria is the production of beta-lactamase enzymes that have the ability to inactivate beta-lactam antibiotics. The blaCMY gene is one of the key genes in this field, which causes resistance to broad-spectrum cephalosporins. Given the increase in resistant strains, the need for alternative strategies to control this resistance seems essential. One of the new strategies to reduce drug resistance is the use of probiotics, which can affect pathogenic bacteria through different mechanisms. The probiotic Lactobacillus reuteri is one of the beneficial and widely used bacteria in the food and medical industries, whose antibacterial effects have been proven in various studies. This bacterium is able to inhibit the growth of Escherichia coli by producing antimicrobial compounds such as rutrin, competing for binding to mucosal cells, and preventing biofilm formation. In this study, the effect of Lactobacillus reuteri on the expression of the blaCMY gene in clinical isolates of Escherichia coli was investigated. The results showed that the presence of this probiotic can lead to a decrease in the expression of this gene and, as a result, reduce resistance to beta-lactam antibiotics. The results of this study showed that the use of probiotics can be effective as a complementary approach in reducing antibiotic resistance. These findings can be used in the design of new therapeutic strategies to combat drug-resistant infections. However, further studies in vitro and clinically are necessary to confirm these effects. Also, investigating the appropriate dose and optimal conditions for the use of Lactobacillus reuteri can help increase its effectiveness in combating resistant bacteria. Finally, expanding research into the use of probiotics as an alternative or complement to antibiotic treatments could help control and reduce the trend of increasing drug resistance.
